Intake Questionnaire
To give your pet the best experience, I like to get to know them first! This questionnaire helps me understand their unique personality, favorite walking pace, and any special needs. By learning their quirks and 'must-haves' ahead of time, I can ensure every walk is safe, fun, and filled with the specialized attention they deserve.
Liability Waiver & Assumption of Risk
Before our first adventure, I require a signed service agreement and liability waiver. This digital document ensures we are on the same page regarding emergency care, veterinary info, and the safety protocols I follow while your pet is in my care. It’s a quick process that you can complete right from your phone or computer. Once signed, we’re all set to focus on the fun stuff—getting those paws in motion!
Veterinary Release Form
Your pet’s health is my highest priority. In the unlikely event of an emergency or sudden illness while your pet is in my care, this form authorizes me to seek immediate veterinary attention if you cannot be reached. It ensures that your preferred vet has the necessary consent to provide treatment, giving us both peace of mind that your furry friend will always receive the care they need without delay
